Change of Insurance General Information

You must supply the office a copy (front and back) 0f your active insurance card before the date of your appointment by one of the following options:

1. If using a smart phone, take a picture and send the pictures VIA email of the front and back of your card to insurance@johnnausmdpa.com

2. Fax a copy of the front and back of your insurance card to 817-545-9134. Bring a physical copy of your card to the office located at 5017 Heritage Avenue, Colleyville, Texas, 76034. Before the date of your appointment.
