You may contact the New Patient Coordinator at (817) 545-9100, option #1 with any questions. Once you have completed the form and submitted it, we will be contacting you about scheduling your appointment.
Step 2.
After completing Step 1., you may need to submit an alternate credit card, the front and back of your insurance card, or an image of your Driver’s License. These changes and new images can be submitted by clicking and completing Step 2. –>
Note: If you submitted these images in Step 1. and there are no corrections, then you do not need to complete Step 2.
Step 3.
Once you have been contacted by our new patient coordinator, your appointment will be scheduled. Please return here to submit your credit card payment information as the final step to secure your appointment time. Your card will not be charged until the 24hrs. prior to your appointment time.
WARNING!
Submission of proof of Insurance is required before the day of your appointment.
Failure to do so will require the full cash pay price to be paid at the time of the appointment.
Most New Patient appointments will be on our virtual telehealth platform and can be reached in our virtual waiting room.
If you are having an in-person appointment:
Arrive at your appointment promptly. Bring the completed New Patient Packet (only if not yet submitted) along with Photo Identification, and your insurance ID card (if applicable).
