You may contact the New Patient Coordinator at (817) 545-9100, option #1 with any questions. Once you have completed the form and submitted it, we will be contacting you about scheduling your appointment.
Step 2.
After completing Step 1., if you want to submit a different card, the front and back of your insurance card or Driver License, this can be done by clicking Step 2. –>
If you submitted these images in Step 1. and there are no corrections then you may skip Step 2.
Step 3.
Once you have been contacted by our new patient coordinator, your appointment will be scheduled. Please return here to submit your credit card payment information as the final step to secure your appointment time. Your card will not be charged until the 24hrs. prior to your appointment time.
WARNING!
Submission of proof of Insurance is required before the day of your appointment.
Failure to do so will require the full cash pay price to be paid at the time of the appointment.
The Day of Your Appointment:
Arrive at your appointment in a timely manner. Bring with you the completed New Patient Packet (if not yet submitted), Photo Identification, and your insurance ID card (if applicable).
Our building is near the intersection of Highway 121 and Glade Road, behind the Bank of America on Glade Road (see the map to the right).
